| Notes: | The data are based on physician fee surveys sent by The Urban Institute to the forty-nine states and the District of Columbia that have a fee-for-service (FFS) component in their Medicaid programs (only Tennessee does not). The Urban Institute derived a Medicaid fee change index to capture fee changes under FFS Medicaid between 2003 and 2008. This index is the weighted sum of the ratios of each service's fee in 2008 to that same service fee in 2003, using the 2000 expenditure weights as in the Medicaid fee index. Because data were available on fewer services and states in 2003 than in 2008, the fee change index was based on a subset of the services and states used in the fee index. |
