Definitions: 

HMO: An entity that offers prepaid, comprehensive health coverage for both hospital and physician services with specific health care providers using a fixed structure or capitated rates. HMO enrollment includes enrollees in both traditional HMOs and HMO point-of-service (POS) plans through: group/commercial plans, Medicare, Medicaid, the Federal Employees Health Benefits Program, direct pay plans and unidentified HMO products.
